Insurance Information

At Jerry E. Lambert, Jr., DDS, PC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information with you to the consultation so that we can expedite reimbursement. If you have any problems or questions, please ask our staff. Upon making your appointment please have your insurance information available so we can verify your benefits before your appointed time.

We also need to keep updated insurance information at all times. Insurance companies do not inform us of any changes that occur to your policy. It is the patients responsibility to keep current information on file. As courtesy we verify insurance information in advance and submit to insurance companies, but we cannot always be aware of any changes made. We ask that you have your insurance information available for any inquiries.
